In-Person Functional Medicine Consult

Service Description

Our Functional Medicine Services are available to people living in the state of Georgia at specific times only. Our next In-Person visits are available March 17-21, 2025. The benefit of this consultation is that a physical exam can be performed during this visit. We make every effort to bill some of your bloodwork may be billed to insurance, you can have medications prescribed to you in addition to supplement/diet and lifestyle recommendations, and you will be eligible to enroll in our Medical Guided Weight Loss program. After you have booked your Functional Medicine Consultation you will receive a link to join our Charm Health portal. You will receive our New Patient Health History, Consent to Treat and Patient Details questionnaires. The Health History asks many questions, please allow yourself time to complete this form. Finally, please add your insurance information and preferred pharmacy. Once your portal is set up, you are able to upload any supporting documents or lab work you would like to review as part of your appointment.
